How to Create a Fillable PDF Form Without Adobe Acrobat

Build a professional fillable PDF form from scratch for free, without installing Adobe Acrobat.

You don't need Acrobat to create forms

Fillable PDF forms are an open standard. PDFQuick Forms creates the same kind of AcroForm fields Acrobat does, in your browser, for free.

Plan the form first

List the information you need and group it into sections: contact details, the request itself, consent. Keep questions short. Decide which fields are required.

Build it

Choose Create a New Form, pick a page size, and add a heading and a sentence of instructions. For each question, add a short label and the matching field: text for names, email and phone fields for contact details, a dropdown for a fixed list of choices, radio buttons for one choice, checkboxes for several.

Make it look professional

Line fields up on a consistent left edge — the editor's alignment guides snap to other elements. Use one font family, keep labels the same size, and leave white space between sections.

Test it in more than one reader

After downloading, open the PDF in your browser and in Adobe Acrobat Reader if you have it. Fill it in and save. Readers differ slightly: Acrobat enforces date formats and required fields; most browsers don't.
